This form is intended to do the following:

• Encourage grantees to thoughtfully reflect on the programs/services that have received funding so that lessons learned can be used to assist in continuous program development.

• Provide information to enable ArtsConnect to evaluate the impact of this funding.

• Gather ideas, perspectives and insights to strengthen future grantmaking decisions.

• Increase the capacity to do constructive and critical review of programs and services.
